Efficacy of Placenta Accreta Index in Predicting Morbidly Adherent Placenta: A Retrospective Analysis

ABSTRACT
Background: Morbidly adherent placenta, including placenta accreta, increta, and percreta, presents significant risks to maternal health and requires meticulous antenatal planning for optimal management. The Placenta Accreta Index (PAI) has emerged as a potential tool for predicting the likelihood of morbidly adherent placenta prenatally.
Aim: This retrospective analysis aimed to assess efficacy of Placenta Accreta Index in forecasting morbidly adherent placenta in a cohort of pregnant individuals.
Methods: Medical records of pregnant individuals diagnosed with morbidly adherent placenta who underwent prenatal imaging and subsequent delivery at our institution between January 2024 to January 2023 were retrospectively reviewed. The Placenta Accreta Index scores, based on prenatal imaging findings including ultrasound and magnetic resonance imaging (MRI), were calculated for each case. The precision of PAI in predicting morbidly adherent placenta was assessed by means of statistical analysis.
Results: A total of 120 cases of morbidly adherent placenta were included in the analysis. These results indicate that the Placenta Accreta Index (PAI) demonstrates good sensitivity and specificity across different value ranges, with higher specificity observed at PAI values above 0.75. The PAI can effectively aid clinicians in identifying women at risk of morbidly adherent placenta, thus facilitating appropriate management and reducing maternal morbidity and mortality associated with this condition.
Conclusion: The findings of this retrospective analysis suggest that the Placenta Accreta Index shows promise as a predictive tool for identifying morbidly adherent placenta prenatally. More probable studies having greater sample sizes are warranted to confirm those findings and refine medical utility of PAI in antenatal management.
